CLIENT GROUPS SERVED:
Per DHS 88.03(2)(b)2, Right Choice Homecare, LLC will be willing to accept and serve clients who are Advanced Aged, Developmentally Disabled, Terminally Ill, or Persons with Traumatic Brain Injury.
The home is a 3-bedroom single-family home, well-equipped, located at the above-stated address, and accessible to individuals without mobility problems. In other words, the home will serve those who are ambulatory or mentally capable of responding to an electronic fire alarm and exiting the facility with or without help or verbal or physical prompting. Details of all services provided by Right Choice Homecare are listed on pages 2-6 of this program statement under “Program Goals and Services.”
In addition, a list of community resources available in the neighborhood, such as recreational centers, shopping malls, and many more places, will also be available to the resident(s) during admission.
The owners, Mohamed Shire and Abdulaziz Farah, will complete pre-admission assessments to ensure clients’ compatibility with the home. All staff will also be trained to ensure they are knowledgeable and experienced in caring for the above-stated client group.
The home will also encourage clients to participate in community activities such as attending the Adult Day Program, church programs, and amusement parks as choices for residents.

We, the Administration and staff of Right Choice Homecare, LLC, believe each resident is a unique, valued individual composed of body, mind, and spirit.
We strive to provide quality services that will facilitate and support behaviors that maintain health and independence, promote quality of life, and preserve individual dignity.
We strive to foster a family-oriented home environment where residents share experiences, find companionship, safety, comfort, and stimulation, and where each resident, family member, friend, and staff person is respected and valued.
NAME OF LICENSEE (OWNER):
Right Choice Homecare, LLC is the licensee owned by Mohamed Shire and Abdulaziz Farah. The owners will be available on-site from every Mon–Sat, 6:00am to 6:00 pm, and every other Sunday same time. When not on-site, the owner is available by phone. Right Choice Homecare employees are available on all shifts to manage the facility. All staffs are highly trained and are the hallmark of our exceptional program.
FEES:
The monthly rate currently ranges from $8,000 to $9,100, depending on the level of care. The fee may be adjusted based on the individual resident’s needs. For example, the payment may differ for those needing increased supervision and assistance. If the monthly fee changes, a 30-day notice will be given.
ENTRANCE FEE:
There is no entrance fee.
ADMISSIONS POLICY:
Our target group includes Advanced Aged, Developmentally Disabled, Terminally Ill, and Person with Traumatic Brain Injury. All the client groups served are elderly. We have found that different client groups within the same facility provide a greater spectrum of people and more diversity to allow interactions for all our residents. The administration will do a preadmission assessment to determine the appropriateness of this facility. The facility will not accept an individual who requires more than 7 hours of nursing care per week.
Residents and employees will be screened for infectious diseases before occupancy or employment. Medical verification of such must be submitted in writing.
PROGRAM GOALS AND SERVICES:
Our goal is to provide an alternative for the elderly who cannot reside alone yet are not nursing home candidates. The Resident’s health, safety, and well-being are our Focus.We provide an environment conducive to a lifestyle of comfort, enjoyment, dignity, satisfaction, privacy, and all the quality things life offers.
Our services are designed with each Resident in mind, attempting to encourage the most significant degree of independence possible. Individual care plans are continually monitored and modified to ensure we reach these goals.
Supervision and Supportive Services include:
The facility provides 24 hours staff members to monitor all resident and staff operations. The facility’s management believes it is essential to have a registered nurse on staff and available 24/7 for all health concerns. The owner will continuously be available to investigate and resolve any grievances. The owner will also give the client/family an annual written update on the resident and their care. In addition, case conferences are offered for the family, patients, guardians, and social workers every six months upon request and with a change in the client’s condition.
Supervision / Assistance with activities of daily living or personal care will be provided when needed. 1 Medication Tech/Caregiver will be staffed on AM shifts, 1 Caregiver/Med Tech will be staffed on PM shifts, and 1 Awake Caregiver/Med Tech will be available to residents on the 3rd shift. The caregiver staff will notify the owner upon a change in the resident health status or any significant events about the resident. All staff has had a thorough employment screening, including extensive background checks. All staff meets the requirements of DHS training. All staff receives up-to-date training and current education by a Registered Nurse.
The Caregiver will be responsible for preparing delicious homemade meals. All residents receive three meals and snacks daily. Special dietary requirements are applied. Meals are served to the resident in family style. Mealtimes are 8am, 12 noon, and 5pm.
For residents who wish to avoid having their meals at the mealtimes listed in the program statement, caregivers will ensure their meals are kept for them until they are ready and willing to have their meals. All residents have the choice of when they want their meals. There will be no restriction whatsoever.
The staff provides an exceptionally active recreational activity program, including onsite activities and outings for resident stimulation. Leisure Activities will include but are not limited to music, journals, newspapers, books, games, crafts, shopping outings, movie outings, TV, sight-seeing outings, activity outings (i.e., concerts, plays, parks), singing along, religious services, birthday parties, holiday celebrations, educational activities, exercise therapy, reality orientation.
We ensure daily cleanliness and sanitation of our home. Laundry and ironing services are also provided. The family must care for clothes requiring dry cleaning. Residents will be responsible for personal toiletry product expenses.
Health monitoring services will coordinate physician care annual exams and as-needed visits. Information and referral services will be provided based on the needs of the residents.
Medication Management is available through our Pharmacy provider. The resident may utilize a pharmacy of their choice. All medications are packaged with a unique “Medication on Time” system. A qualified Pharmacist provides services and education.
All residents must use an approved medication system to ensure safety and efficacy. Staff will only administer medications with administrative and physician approval. Staff members are trained and certified in medication administration before dispensing medicines or treatments.
Transportation arrangements will be made with the County Ride line, hospital services, or ambulance services when families cannot provide this service to health care appointments. Transportation for leisure activities and religious activities will be provided by the family, social worker, or approved ride system (Transit Plus, DCS, and Americare).
Hair styling outing is at the resident’s cost. Daily grooming supervision/assistance will be provided by regular facility staff.
Emergency preparedness – Policies will be available for staff and residents, and periodic communication with city and county agencies associated with emergency activities (local fire and police).
Banking – Residents are encouraged to keep only a small amount of cash ($5.00) on-site. This money will be locked up until the resident requests it.
Communication with residents, staff, family members and friends, and other community agencies is of utmost importance to achieve a satisfactory lifestyle for the resident. Families are encouraged to visit frequently.
Outside intermittent nursing or medical care can be arranged for the resident at their expense. Supportive home or personal care services will always be provided, and nursing care of at most 7 hours per week.
ADVANCE DIRECTIVES:
Staff shall follow the advance directives set by the resident. It will not be a requirement for admission or a condition of receiving any health care service.
RESPITE CARE:
Depending on availability, one or more beds may be set aside for respite care. Respite care is provided for up to 14 consecutive days.
ADMISSIONS AND RETENTION:
The home is equipped to serve the following primary client groups: Advanced Aged, Developmentally Disabled, Terminally Ill, and Person with Traumatic Brain Injury. The home offers a clean, calm, and conducive atmosphere. The services available are:
a) Administration of tube feedings, medications, and insulin by RN or LPN
b) Providing nutritious meals with special diets as ordered by the physician and Registered Dietician. Certified Nursing Assistant, PCW, and Med Tech will provide the service.
c) Monitoring resident’s health by observing and documenting changes in condition and referring resident to health care providers, when necessary, by PCW, RN, or LPN.
d) Supervising or assisting the resident with or teaching the resident about activities of daily living (ADL) by PCW, RN, or LPN.
e) Notification of the resident’s guardian, family, and placing agency of any significant changes in a resident’s condition by RN or LPN.
f) Ensuring annual health examinations by a physician are done. RN or LPN will monitor this.
g) Arranging for transportation to activities, employment, and appointments. Done by the Owner or Caregiver.
h) Keeping a current record of all medical visits, reports, and recommendations for the resident. Done by RN or LPN.
i) Resident and family involvement in plan of care, care conferences, field trips to special events when applicable.
j) PT/OT and Speech therapy as needed in-house or outpatient services. Done by PT/OT/ST.
k) Community resources will also be made available to residents periodically as needed.
Appropriate bedding and linens shall be maintained in a clean condition. When the waterproof mattress is used, the facility will ensure a washable mattress pad, the same size as the mattress, over the waterproof mattress cover. Residents are responsible for medication, personal products, and clothing costs.
Hospice Care
Persons diagnosed with a terminal diagnosis may be considered for Hospice Care. A hospice company may be brought into the facility. Residents requiring Hospice Care must have a state-approved Hospice team managing their health, medications, and comfort level. Approved Hospice agencies must be available 24 hours per day. Such agencies include, but are not limited to, Vitas, Asera Care, and Horizon Homecare.
